Co-Insurance


Coverage Description
Co-insurance is the percentage of Eligible Expenses, after the deductible, which is the responsibility of the insured. For example, if an insured had a $1,250 claim with an 80/20 co-insurance benefit and a $250 deductible, the insured would be responsible for $450 ($250 deductible plus 20%) and the insurance company would pay the remaining amount of $800.

The term “Coinsurance” shall mean the percentage amount of eligible Covered Expenses, after the Deductible, which are the responsibilities of the Insured Person and must be paid by the Insured Person. The Coinsurance amount is stated in Section II, Schedule of Benefits, under each stated benefit.

f the Insured Person is traveling inside the United States and Canada: When a covered Injury or Illness is incurred by the Insured Person, the Company will pay 80% of the first $5,000 of Reasonable and Customary medical charges for Covered Expenses, excess of the Policy Period Deductible as stated on the ID Card. Thereafter, the Company will pay 100% of Reasonable and Customary medical charges for Covered Expenses up to the medical maximum as stated on the ID Card.

If the Insured Person is traveling outside the United States and Canada: the Company will pay 100% of Reasonable and Customary medical charges for Covered Expenses, excess of the Policy Period Deductible as stated on the ID Card, up to the medical maximum as stated on the ID Card. In no event shall the Company's maximum liability exceed the medical maximum as stated on the ID Card. The Deductible and Coinsurance amount consists of Covered Expenses which would otherwise be payable under this Policy. These expenses must be borne by each Insured Person. A maximum of 3 Policy Period deductibles per family under the same application will apply.

The Company will pay up to the benefit limit for covered expenses incurred outside the USA during the period of coverage which are the direct result of an Injury or Sickness. Covered medical expenses are necessary services and supplies which are recommended by the attending Physician.

The Company will not pay benefits in excess of Reasonable and Customary charges commonly used by providers of medical care in the locality in which the care is furnished.

For the Certificate Period, underwriters will pay 80% of the next $5,000 of Eligible Expense after the Deductible, then 100% up to the Overall Maximum Limit.

Coinsurance will be waived if expenses are incurred within the PPO and expenses are submitted to Underwriters for review and payment directly to the provider.

Claims incurred outside US or Canada

For the Certificate Period, underwriters will pay 100% of the Eligible Expenses after the Deductible up to the Overall Maximum Limit.
