What is Pre-Existing Condition?

A Pre-Existing Condition is an illness or other medical condition that was treated, diagnosed, or changed during a policy’s lookback period.

To determine what is considered a Pre-Existing Condition, providers look back between 60-180 days prior to the day the policy was purchased. If a traveler had any changes to their medical status during that period, including a new diagnosis, a decline in health, or a change in prescription, that condition will be considered pre-existing.

Pre-Existing Conditions are typically excluded by travel insurance, but that exclusion can be waived by some policies. Pre-Existing Condition coverage is time-sensitive, meaning it is typically only available for 14-21 days after a traveler makes their initial trip deposit.

We will not pay for loss caused by or resulting from:

1. Pre-Existing Conditions, unless: a) the Policy is purchased within 14 days of Your initial trip deposit; b) the booking for the Covered Trip must be the first and only booking for this travel period and destination; and c) You are not disabled in a way that prevents you from travelling at the time You pay the premium;

In addition to the exclusions in the Policy, the following exclusions apply to Emergency Medical Evacuation and Medically Necessary Repatriation coverage:

1. Pre-Existing Conditions, to the extent they are excluded in Your base Policy;

“Pre-Existing Condition” means any Accidental Injury, sickness or condition of You, Your Traveling Companion or Your Family Member booked to travel with You for which medical advice, diagnosis, care or treatment was recommended or received within the 180 day period ending on the Effective Date. 1. a Pre-Existing Medical Condition, as defined in the policy;

Death resulting from a Pre-Existing Medical Condition will not be excluded. Death must occur prior to the termination date of the benefit under which the claim is being made.

PRE-EXISTING MEDICAL CONDITION EXCLUSION WAIVER

We will waive the Pre-Existing Medical Condition exclusion if all of the following conditions are met:

a) Your premium for this policy is received within the Time Sensitive Period.

Time Sensitive Period means insurance must be purchased at or before the final payment for Your Trip.

Pre-Existing Medical Condition means an illness, disease, or other condition during the sixty (60) -day period immediately prior to the date Your coverage is effective for which You or Your Traveling Companion, Business Partner, Family Member, Pet, or Service Animal scheduled or booked to travel with You:

1. received or received a recommendation for a test, examination, or medical treatment for a condition which first manifested itself, worsened or became acute, or had symptoms which would have prompted a reasonable person to seek diagnosis, care or treatment; or

2. took or received a prescription for drugs or medicine. Item (2) of this definition does not apply to a condition which is treated or controlled solely through the taking of prescription drugs or medicine and remains treated or controlled without any adjustment or change in the required prescription throughout the sixty (60) day period before coverage is effective under this policy.

3. required a change in prescribed medication. Change in prescribed medication means the dosage or frequency of a medication has been reduced, increased, stopped and/or new medications have been prescribed due to the worsening of an underlying condition that is being treated with the medication, unless the change is:

a) between a brand name and a generic medication with comparable dosage; or
b) an adjustment to insulin or anti-coagulant dosage.

b) You insure all Prepaid Trip costs that are subject to cancellation penalties or restrictions and also insure within 20 days of the Payment or Deposit for those Travel Arrangements the cost of any subsequent Travel Arrangements (or any other Travel Arrangements not made through Your travel agent) added to Your Trip; and

c) You are not disabled from travel at the time Your premium is paid.

Pre-Existing Condition means any injury, sickness or condition of You, an Insured’s Traveling Companion, You or an Insured’s Traveling Companion’s Family Member for which within the sixty (60) day period prior to the effective date of Trip Cancellation coverage under the Group Policy such person received diagnosis or treatment for such injury, sickness or condition.

a) Pre-Existing Condition(s) except as waived under Sections 3.9 and 5.1 through 5.5 above;

Acute Onset of Pre-Existing Condition(s):

If you are under the age of sixty-five (65) years, the exclusion set forth in Section 8(a) does not apply to the eligible medical Expenses for the first Acute Onset of a Pre- Existing Condition(s) during the Period of Coverage, up to the amount set forth in the Schedule of Benefits for eligible medical Expenses incurred outside of Your Home Country. This benefit also applies to Insured Persons from age sixty-five (65) years to seventy-nine (79) years for the amount set forth for them in the Schedule of Benefits. The Deductible, Copay and Coinsurance options set forth in Section 3.1 apply to this coverage and will be Your responsibility.

This benefit does not include coverage for known, scheduled, required, or expected medical care, drugs, or Treatments existent or necessary prior to departure from Your Home Country and prior to the Effective Date of Coverage; coverage for Treatment for which You have traveled; or coverage for conditions for which travel was undertaken after Your Physician has limited or restricted travel.

Coverage begins 72 hours (3 days) after the Effective Date of Coverage and ceases on the earliest of:

(i) The condition no longer being considered acute; or
(ii) Your discharge from the Hospital.

As set forth in the Schedule of Benefits, there is no coverage under this benefit for Insured Persons age eighty (80) years and over.

Acute Onset of a Pre-Existing Condition(s): Sudden and unexpected outbreak or recurrence of a Pre-Existing Condition(s) is one:

a) That occurs spontaneously and without advanced warning either in the form of Physician recommendations or symptoms and requires urgent care;

b) That occurs during the Period of Coverage, after the 72-hour (3-day) waiting period, and prior to the age shown in the Schedule of Benefits, as indicated in Section 3.9; and

c) For which Treatment is obtained within twenty-four (24) hours of the sudden and unexpected outbreak or recurrence.

A Pre-Existing Condition will not be considered an Acute Onset of a Pre-Existing Condition(s):

(i) If the condition is chronic, Congenital or gradually becomes worse over time; or

(ii) If, during the thirty (30) days prior to the acute event, You had a change in prescription or Treatment for a diagnosis related to the underlying Pre-Existing Condition.

Pre-Existing Condition(s):

Any medical condition, sickness, Injury, Illness, disease, Mental Illness or Mental or Nervous Disorder, regardless of the cause, including any Congenital, chronic, subsequent, or recurring complications or consequences related thereto or resulting therefrom, that with reasonable medical certainty existed at the time of Application or any time during the thirty-six (36) months prior to the Effective Date of Coverage under this Certificate, whether or not previously manifested, symptomatic, known, diagnosed, treated, or disclosed. GENERAL LIMITATIONS AND EXCLUSIONS

In addition to any applicable benefit-specific exclusions, the following exclusions apply to all losses and all benefits. Unless otherwise shown below, these exclusions apply to you, your traveling companion, family member and host at destination. This policy does not cover any loss for, caused by or resulting from:

aa. Any loss or expense incurred as the result of a pre-existing medical condition.

PRE-EXISTING MEDICAL CONDITION EXCLUSION WAIVER

We will waive the pre-existing medical condition exclusion if the following conditions are met:

a. This plan is purchased within fifteen (15) days of initial trip payment;
b. The amount of coverage purchased equals all prepaid nonrefundable payments or deposits applicable to the trip at the time of purchase and the costs of any subsequent arrangements added to the same trip are insured within fifteen (15) days of initial trip payment for any subsequent trip arrangements;
c. All insureds are medically able to travel when this plan cost is paid; and
d. The trip cost does not exceed $20,000.

Trip Cancellation Exclusions:

In addition to the General Limitations and Exclusions, the following exclusions apply to the Trip Cancellation Benefit. No benefits will be paid for any loss for, caused by, or resulting from:

k. Pre-Existing medical condition exclusion: We will not pay for any loss or expense incurred as the result of an injury, sickness or other condition (including any condition from which death ensues) of you, your traveling companion, host at destination, business partner or family member which, within the one hundred eighty (180) day period immediately preceding and including your coverage effective date: a) first manifested itself, worsened, became acute or had symptoms which would have prompted a reasonable person to seek diagnosis, care or treatment; b) for which care or treatment was given or recommended by a physician; c) required taking prescription drugs or medicines, unless the condition for which the drugs or medicines are taken remains controlled without any change in the required prescription drugs or medicines.

PRE-EXISTING MEDICAL CONDITION EXCLUSION WAIVER

The Company will waive the pre-existing medical condition exclusion if the following conditions are met:

a. This plan is purchased within fifteen (15) days of initial trip payment;
b. The amount of coverage purchased equals all prepaid nonrefundable payments or deposits applicable to the trip at the time of purchase and the costs of any subsequent arrangements added to the same trip are insured within fifteen (15) days of the date of payment or deposit for any subsequent trip arrangements;
c. All insureds are medically able to travel when this plan cost is paid; and
d. The trip Cost does not exceed $20,000, per person (only applicable to trip Cancellation/Interruption/ Delay).

This coverage will be terminated and no benefits will be paid under this pre-existing medical condition exclusion waiver coverage if the full costs of all prepaid, non-refundable trip arrangements are not insured.

Trip Interruption Exclusions:

In addition to the General Limitations and Exclusions, the following exclusions apply to the Trip Interruption Benefit. No benefits will be paid for any loss for, caused by, or resulting from:

j. Pre-Existing medical condition exclusion: We will not pay for any loss or expense incurred as the result of an injury, sickness or other condition (including any condition from which death ensues) of you, your traveling companion, host at destination, service animal, business partner or family member which, within the one hundred eighty (180) day period immediately preceding and including your coverage effective date: a) first manifested itself, worsened, became acute or had symptoms which would have prompted a reasonable person to seek diagnosis, care or treatment; b) for which care or treatment was given or recommended by a physician; c) required taking prescription drugs or medicines, unless the condition for which the drugs or medicines are taken remains controlled without any change in the required prescription drugs or medicines.

In addition to the General Limitations and Exclusions, the following exclusions apply to the Accidental Death and Dismemberment – Common Carrier (Air Only) Benefit. No benefits will be paid for any loss for, caused by, or resulting from:

i. Pre-existing medical conditions.

Pre-existing medical condition means an injury, sickness, death or other condition of you, your traveling companion or a family member, to which any of the following applied within the one hundred eighty (180) day period immediately preceding and including the purchase date of this plan:

a. First manifested itself, worsened, became acute or had symptoms which would have prompted a reasonable person to seek diagnosis, care or treatment, or;
b. Care, testing or treatment was given or recommended by a physician; or
c. Required a change in prescribed medication.

Change in prescribed medication means the dosage or frequency of a medication has been reduced, increased, stopped and/or new medications have been prescribed due to the worsening of an underlying condition that is being treated with the medication, unless the change is:

a. Between a brand name and a generic medication with comparable dosage; or
b. An adjustment to insulin or anti-coagulant dosage.

Pre-Existing Conditions Waiver

We will waive the Pre-Existing Conditions exclusion provided:

a. The Policy is purchased within 14 days of Your Initial Trip Deposit;
b. The booking for the Covered Trip must be the first and only booking for this travel period and Destination;
c. You are not disabled in a way that prevents You from travelling at the time You pay the premium;
d. You are medically able to travel when the plan is purchased; and
e. You insure 100% of Your Trip Cost.

We will not pay for Loss caused by or resulting from:

1. Pre-Existing Conditions, unless the Pre-Existing Conditions Waiver is met;

Pre-Existing Condition means any Accidental Injury, Sickness or condition of You, Your Traveling Companion or Your Immediate Family Member booked to travel with You for which medical advice, diagnosis, care or treatment was recommended or received within the 90 day period ending on the Effective Date. Sicknesses or conditions are not considered pre-existing if the Sickness or condition for which prescribed drugs or medicine is taken remains controlled without any change in the required prescription throughout the entire 90 day period ending on the Effective Date and no medical advice, diagnosis, care or treatment has otherwise been received.

Waiver of the Pre-Existing Condition Exclusion

The exclusion for Pre-Existing Condition will be waived provided:

a) Your Payment for this Policy and enrollment form are received within 21 days of the date Your initial Payment or Deposit for Your Trip is received; and

b) You insure all Prepaid Trip costs that are subject to cancellation penalties or restrictions and also insure within 21 days of the Payment or Deposit for those Travel Arrangements the cost of any subsequent Travel Arrangements (or any other Travel Arrangements not made through Your travel agent) added to Your Trip; and

c) You are not disabled from travel at the time Your premium is paid.

2) took or received a prescription for drugs or medicine. Item (2) of this definition does not apply to a condition which is treated or controlled solely through the taking of prescription drugs or medicine and remains treated or controlled without any adjustment or change in the required prescription throughout the 60 day period before coverage is effective under this policy.

3) Change in prescribed medication means the dosage or frequency of a medication has been reduced, increased, stopped and/or new medications have been prescribed due to the worsening of an underlying condition that is being treated with the medication, unless the change is:

“Unexpected Recurrence of a Pre-Existing Condition” means a sudden and unexpected outbreak or recurrence of a Pre- Existing Condition which occurs 1) spontaneously and without advance warning either in the form of Physician recommendations or symptoms, is of short duration, is rapidly progressive, and requires urgent and immediate medical care; 2) is a minimum of 48 hours after the Effective Date of the policy; and 3) prior to the age shown in the Schedule of Benefits/Limits with treatment being obtained within 24 hours of the sudden and unexpected outbreak or recurrence. A Pre-Existing Condition that is a congenital condition or that gradually becomes worse over time will not be considered an Unexpected Recurrence.

ADDITIONAL MEDICAL TREATMENT AND SERVICES

Unexpected Recurrence of a Pre-Existing Condition – Benefits are payable for Covered Expenses resulting from a sudden, Unexpected Recurrence of a Pre-Existing Condition while traveling outside the Covered Person’s Home Country. This benefit does not include coverage for known, scheduled, required, or expected medical care, drugs or treatments existent or necessary prior to the Effective Date of coverage.

“Pre-Existing Condition” means Any Injury, illness, Sickness, disease, or other physical, medical, mental or nervous disorder, condition or ailment that, with reasonable medical certainty, existed at the time of Application or at any time during the 36 months prior to the Effective Date of this insurance, whether or not previously manifested, symptomatic or known, diagnosed, Treated, or disclosed to the Company prior to the Effective Date, and including any and all subsequent, chronic or recurring complications or consequences related thereto or resulting or arising therefrom. This specifically includes but is not limited to any medical condition, Sickness, Injury, illness, disease, mental illness or mental nervous disorder, for which medical advice, diagnosis, care or Treatment was recommended or received or for which a reasonably prudent person would have sought Treatment during the 36 month period immediately preceding the Effective Date of Coverage under this Certificate. A Pre-Existing Condition which is a chronic or congenital condition or that gradually becomes worse over time and/or known, scheduled, required, or expected medical care, drugs or Treatments existing or necessary prior to the Effective Date are not considered to be an Acute Onset.

ACUTE ONSET OF PRE-EXISTING CONDITION

For ages up to and including 69 the limit is up to the Medical Policy Maximum purchased per Period of Coverage except for any coverage related to cardiac disease or conditions, which will be limited to $25,000 up to and including age 69 and $15,000 for ages 70 and above. Upon attaining age 70 Acute Onset benefits will be reduced to a Maximum of $35,000, with a $25,000 Maximum Lifetime Limit for Emergency Medical Evacuation.

ACUTE ONSET OF PRE-EXISTING CONDITION (PER POLICY PERIOD) Benefits are payable for an Acute Onset of a PreExisting Condition. Any repeat/reoccurrence within the same Policy Period will no longer be considered Acute Onset of a Pre-Existing Condition and will not be eligible for additional coverage. A Pre-Existing Condition which is a chronic or congenital condition or that gradually becomes worse over time and/or known, scheduled, required, or expected medical care, drugs or treatments existing or necessary prior to the Effective Date are not considered to be an Acute Onset. This benefit covers only one (1) Acute Onset episode of a Pre-Existing condition. Sudden and Acute Onset of a Pre-Existing Condition Coverage expires upon medical advice that the condition and onset is no longer acute, or the Covered Person is discharged from a medical facility.

“Acute Onset of a Pre-Existing Condition” Acute Onset of a Pre-Existing Condition means a sudden and unexpected outbreak or recurrence of a Pre- Existing Condition which 1) occurs spontaneously and without advance warning either in the form of Physician recommendations or symptoms, is of short duration, is rapidly progressive, and requires urgent and immediate medical care; and 2) occurs a minimum of 48 hours after the Effective Date of the policy and 3) treatment is obtained within 24 hours of the sudden and unexpected outbreak or recurrence. A Pre-Existing Condition that is a Congenital condition or that gradually becomes worse over time will not be considered Acute Onset.

Excluded Conditions, Treatments (includes Diagnoses, Tests, and Examinations), Services, Supplies, Acts, Omissions, and/or Events:

1. Pre-existing Conditions, except charges resulting directly from an Acute Onset of Pre-existing Condition, as herein defined, subject to the limits set forth in the Schedule of Benefits and Limits.

PRE-EXISTING MEDICAL CONDITIONS

A Pre-Existing Condition is an illness or other medical condition that was treated, diagnosed, or changed, or existed with reasonable medical certainty during a policy’s lookback period.

This policy does not cover pre-existing conditions, except charges resulting directly from an Acute Onset of Pre-existing Condition subject to the limits set forth in the Schedule of Benefits and Limits.

Pre-existing Condition means any

1. Condition for which medical advice, diagnosis, care, or treatment (includes receiving services and supplies, consultations, diagnostic tests or prescription medicines) was recommended or received during the 2 years immediately preceding the certificate effective date; or

2. Condition that had manifested itself in such a manner that would have caused a reasonably prudent person to seek medical advice, diagnosis, care, or treatment (includes receiving services and supplies, consultations, diagnostic tests or prescription medicines) within the 2 years immediately preceding the certificate effective date; or

3. Injury, illness, sickness, disease, or other physical, medical, mental, or nervous conditions, disorder or ailment (whether known or unknown) that, with reasonable medical certainty, existed at the time of application or within the 2 years immediately preceding the certificate effective date. For the purposes of the Complications of Pregnancy coverage offered hereunder, pregnancy will not be included within the definition of a pre-existing condition.

ACUTE ONSET OF PRE-EXISTING CONDITION

YOU ARE COVERED:

1. Charges for a sudden and unexpected outbreak or recurrence of a pre-existing condition(s) which:

a. Occurs when you are under age 80; and
b. Occurs spontaneously and without advance warning either in the form of physician recommendations or symptoms; and
c. Is of short duration; and
d. Is rapidly progressive; and
e. Requires urgent care.

YOU ARE NOT COVERED unless you fulfill the following condition:

1. Treatment must be obtained within 24 hours of the sudden and unexpected outbreak or recurrence.

YOU ARE NOT COVERED IF:

1. The Acute Onset of a Pre-existing Condition(s) occurs before the certificate effective date; or

2. The pre-existing condition is

• a chronic or congenital condition; or
• a complication or consequence of a chronic or congenital condition; or
• a condition that gradually becomes worse over time;
or

3. The charges are for known, scheduled, required, or expected medical care, drugs or treatments existent or necessary prior to the certificate effective date; or

4. Expenses arise directly or indirectly from anything in the General Exclusions.
